Logan-Hocking Local School District

The Hocking Valley Community Hospital Chieftain Health Clinic will be opening soon.  The clinic has been built as an addition to Logan High School but is available to all students in our district.   Families can use the Chieftain Health Clinic just as they would an urgent care. Your child will be coming home this week with paperwork for the Chieftain Health Clinic.  This includes a welcome letter, registration form, health history, and consents.  Please check your child's bookbag for this paperwork and return to your school if you choose to do so.  The paperwork will be collected and stored at the Chieftain Health Clinic, but no student will be seen in the clinic without verbal consent from the parent or guardian first.  Returning this completed paperwork is optional but will streamline the process if your family chooses to use the clinic.  We look forward to the opening of the clinic and the ability to provide additional access to healthcare for our students. Families will be notified when the clinic opens.
