Welcome to Chieftain Health Clinic!

A school-based health clinic to serve students and staff conveniently located on site at Logan High School

Hocking Valley Community Hospital Chieftain Health Clinic was created from a partnership between Hocking Valley Community Hospital and Logan-Hocking School District to remove barriers such as transportation, missed school or delays, and improve access to healthcare for students in Hocking County. By serving students on site at the High School, our goal is to reduce missed school and work hours for both children and their families, providing access to quality healthcare and thereby improving children’s academic success and overall health. We serve students of all ages, grades preschool through 12. Transportation is available free of charge to elementary and middle school students for well visits, vaccinations and other services.

Services Offered at the Chieftain Health Clinic Include:
● Treatment of acute illness such as sore throat, sinus infection, flu, and common cold
● Chronic Disease management such as asthma and diabetes
● Sports and Pre-Employment Physicals
● Comprehensive annual wellness visits
● On Site Testing such as blood sugar, flu, strep, and urinalysis.
● Vaccinations

Parental or guardian consent is required for each visit. We welcome and encourage parents to join us in
the clinic for their student’s visit, however, we know that may not always be possible. In this instance, a signed
consent for treatment must be on file prior to their visit. You may also choose to give permission for another
adult to accompany your student to their visit.

All visits will be billed to your insurance. As with any healthcare service, the cost will vary depending on the
complexity of the visit and any testing provided. If you need financial assistance, please let us know and we will
be happy to assist you.
Please be sure to bring the following to your visit:

○ New Patient Services Consent Form ○ Your insurance card(s) ○ Any required custody paperwork
